Dr. Matthew Smith has treated more than ten thousand complex spinal cases from a South Jordan clinic. He says the reason the practice works is the patients he turns down.

Ask Dr. Matthew Smith what a great day at work looks like and he doesn't reach for a dramatic recovery story. He describes a man tying his shoes.

“Someone puts their shoes on without sitting down first,” he says. "That sounds small. It isn't." A patient who hasn't been able to bend over in three years walks into the waiting room, ties his own laces, and doesn't even notice he did it. “That's the day. Not the big dramatic moment — the ordinary thing they'd stopped expecting to get back.”

Smith graduated from Parker College of Chiropractic in Dallas in 2002 and came home to Utah, where he'd grown up playing football and basketball at Highland High. But the part of his education that actually shaped how he practices happened well outside a classroom. Fifty-four marathons. Three Ironmans. A certification as a strength and conditioning specialist through the National Strength and Conditioning Association, and years spent studying how a body breaks down under heavy load and how it comes back.

Somewhere in there, a question lodged itself and wouldn't leave.

“Why does a body that recovers from a hundred-mile training week not recover from one bad disc?”

The answer he landed on was pressure and blood supply. A spinal disc held under constant compression can't draw in the water and nutrients it needs to repair itself. Rest doesn't fix it. Neither does masking the pain. So Smith began building a system around two goals: take the pressure off, and speed the repair up. More than two decades and ten thousand complex cases later, that system is what the clinic runs today. He calls it LaZR-DCoM.

Four things, all at once

Smith is precise about what the protocol requires, and he's blunt that it collapses if any one piece is missing.

The first is a certified team that has run the protocol hundreds of times. The second is genuine medical-grade decompression — "not a stretch table," he says, drawing a hard line between the equipment his clinic uses and the entry-level machines that have proliferated across the market. The third is a true Class IV therapeutic laser, the deep-tissue class of device used in clinical research settings rather than the low-power units sold for home wellness use.

The fourth is the one nobody likes.

“We only accept the cases we know will respond,” Smith says. The clinic tracks its own outcomes and reports that 92 percent of patients accepted into the protocol get the result they came for — a number, he says, that exists precisely because of the people he sends away. Roughly half of everyone who walks through the door for an evaluation isn't a candidate.

“If I took everybody, I'd help fewer people, not more.”

Age is not a diagnosis

The clinic's oldest patient is ninety-nine. She had been told her whole problem was simply age, and that at her stage of life the answer was to live with it. What nobody had done, Smith says, was check whether her tissue could still respond. It could.

"That case is the one I think about when someone in their sixties tells me they're probably too old for this," he says. "Age is not a diagnosis."

Word has traveled further than a South Jordan strip of 4000 West would suggest. Patients have flown in from South Africa, Brazil, Alaska, and across the western United States specifically for treatment.

This year the clinic added a second set of hands: Gonzalo Cabezas, MSN, FNP, who brings more than twenty years in medicine and now focuses on pain management alongside the protocol. The logic is straightforward, Smith says. Pain and structure are two different problems that happen to arrive in the same body. Cabezas can address what hurts right now while the protocol works on what's causing it — so patients aren't asked to simply endure the weeks it takes for tissue to heal.

The no is the point

Smith is unsentimental about what it means to be the last stop.

“People come to us at the end of the line, and they've already been sold hope by somebody,” he says. "The worst thing I could do to a person in that spot is take their money and their last bit of optimism on a case I already know won't respond. So we test, we image, we evaluate, and sometimes I say no."

“That no is the reason the yes means something.”

Ask him what he wants the name Elite Performance to mean on the Wasatch Front ten years from now and he doesn't say biggest.

“I want it to be the place Utah doctors send the patient they can't help,” he says. “If a surgeon in Sandy or a physical therapist in Herriman hits a wall with a patient and their first thought is send them to Elite Performance — that's the whole goal.”

And to the reader who has never met him, who has been carrying something for a year or five and has an appointment on the calendar they're dreading?

“Come find out whether you're a candidate before you agree to anything permanent,” Smith says. “An evaluation costs you an hour. Surgery costs you the rest of your life either way it goes. I've had too many people tell me they wish they'd known there was a step in between. Ask the question first. If we can't help you, I'll tell you that to your face and point you toward who can.”
